In a SOAP draft, identify the particular work you are documenting before assigning headings. A referral call, a psychotherapy intervention, and a discussion of transportation are different activities. A generic sentence such as resources provided does not tell a reader which action occurred or what is still unresolved.

Fictional client reports missing a community appointment because a usual ride was unavailable. During the practice encounter, the social worker reviews two transportation options with the client. With the fictional scenario’s appropriate permission already established, the social worker calls one program; the program provides an application link but does not confirm eligibility. Client says they want to review the application. The worker’s supplied assessment identifies transportation access as an unresolved practical issue. No other service outcome is known.

The example’s plan is an explicit fictional agreement. In a real source, do not add that follow-up unless it was supplied. Permission and disclosure details belong in the authorized record workflow.
Prevent premature closure in the wording
Compare contacted program with connected client to transportation. The first describes an action; the second may imply a completed service connection. Similar problems arise with referred, accepted, enrolled, scheduled, and attended. Choose the verb that matches the latest verified step. If your information comes from the client’s report rather than a provider confirmation, preserve that source. This precision helps the next professional know what to follow up without relying on your memory of the call.
