Two times answer two different questions

The encounter time identifies when the service or event occurred. The entry time identifies when the documentation was created. Confusing them can make a record difficult to interpret. CMS’s cited appendix discusses identifying delayed entries and their authorship in a medical-review setting. Because that source is an older operational document, verify current requirements for your organization and payer. This page does not set a universal deadline or tell you how to cure a documentation problem.

Check the available source before writing

A delayed note may rely on authorized contemporaneous materials and the responsible author’s recall, subject to the practice’s rules. Be clear about what is actually supported. An appointment on a calendar does not establish the interventions performed or the client’s response. A partial worksheet does not prove every planned activity happened. If relevant details cannot be established, do not borrow them from the next visit or copy a customary session paragraph to fill the gaps.

Original fictional chronology exercise

Imagine a fictional session occurred on Monday and the author begins the delayed entry on Wednesday. A contemporaneous practice worksheet records that the therapist reviewed two planning options and the client selected one. It does not record a client response beyond that selection or an agreed follow-up frequency. A source-faithful narrative can preserve the review and selection while leaving unestablished details unresolved. The authorized record system would need to show the actual encounter and entry context according to its delayed-entry process.

Avoid backdating and hindsight

A later event should not be written as though it was known at the original session. Suppose the client reports on Friday that the chosen plan was helpful. That report belongs to its proper later context; it cannot retrospectively become Monday’s observed response. Likewise, do not change an entry timestamp or service date to disguise delay. If a record needs a separate correction, amendment, or follow-up entry, the responsible professional should use the appropriate process rather than combining distinct events into one polished paragraph.
