Data is a container with internal labels
Data can hold reports, observations, interventions, and responses. Combining them under one heading does not make their sources interchangeable. Preserve phrases such as client reported and therapist observed. If someone later reads only the Data paragraph, they should still be able to distinguish an action in the session from a recollection of the week. NASW’s format comparison identifies DAP as a three-part option; individual agencies may require additional fields or another structure.
Example one: a practice task was attempted
Fictional source: client reports using a two-column worksheet once. Therapist and client review the completed worksheet. Client identifies one item they want to revise. The therapist’s supplied assessment says that one attempt is documented, with repeat use not yet established. Client agrees to try the revised worksheet once.

Example two: the response is missing

Keep interpretation proportionate
Watch the verbs in Assessment. Tried, described, completed, and demonstrated refer to different kinds of evidence. A worksheet completed during a session does not establish use at home. A client’s report of a helpful exercise does not prove a broader treatment outcome. If the clinician has a justified interpretation, record it as their interpretation and connect it to its basis. A formatting utility cannot supply that reasoning on behalf of the author.
Build and review your own DAP arrangement

Before export, ensure that planned actions were actually agreed and that empty fields have been resolved through your authorized clinical workflow rather than guessed.

Is DAP shorter than SOAP?
It has fewer headings, but the useful length depends on the encounter and record requirements. Fewer headings do not justify omitting relevant information.
