A modality label is the beginning of a description
CBT identifies a broad treatment approach. A useful record describes the work that actually occurred, as supplied by the professional, without using the label as a substitute for the encounter. This page does not teach cognitive restructuring, determine that CBT is appropriate, or recommend a worksheet. It shows how an author’s existing description can remain precise when converted into a structured note.
Fictional source: one worksheet review
Fictional therapist identifies the session activity as a CBT thought-record review. Client brings a worksheet containing one situation and an initial interpretation. Therapist and client examine that entry. Client adds a second possible interpretation during the session and says they want more practice with the form. The therapist supplies an assessment that a second interpretation was produced with in-session support; independent use has not been established. Client agrees to bring one further practice entry next time.

Make level of assistance visible when supplied
Completed a worksheet and completed a worksheet with therapist prompts do not communicate the same thing. If assistance was part of the source, retain it. A draft that drops the assistance may overstate independent performance. Conversely, do not insert required extensive prompting if the author did not describe that. The relevant editorial task is to preserve the activity conditions, the actual response, and the author’s interpretation, rather than ranking the quality of the client’s performance yourself.
Link the next step to the actual agreement
A worksheet discussed in session should not automatically produce daily homework in Plan. Use the frequency, task, and timing actually agreed, leaving details unresolved when absent. In NoteFrame, put the worksheet review in Interventions and the added interpretation plus request in Response. The assessment remains the therapist’s supplied view. The editor cannot measure CBT fidelity, score a worksheet, or decide that a cognitive change occurred. Its value is a legible draft with a visible path back to the facts the author entered.

No. It does not analyze client thoughts or classify them. Enter only interpretations supplied by the responsible professional.
Should the whole worksheet be copied into the note?
Decide what is relevant and required in your practice. This example demonstrates a concise account of the activity and response rather than automatic full transcription.
